~ ~ REGISTRATION REQUIREMENTS~ ~

PROOF OF RESIDENCY AND ELIGIBILITY:

v Please bring original documents and one copy for school files

Ø An original birth certificate for the child you are registering must be presented.

Ø If you own your home, you must present your mortgage statement, property tax receipt and three bills printed with your name and address (e.g., phone bill, LIPA bill, water bill, cancelled personal check with imprinted address, printed paycheck to the address of residence or court issued documentation).

Ø If you are renting or living with others, the elementary school will provide you with an affidavit which must be notarized (it can be downloaded from the district website). In addition, the owner of the property must provide you with the following: tax receipt and mortgage statement or home insurance and three utility bills which must also be submitted at time of registration. You will also have to provide proof of address by presenting two additional bills received in your name at this address (e.g., credit card, phone, LIPA, cancelled personal check with imprinted address, food stamp number or printed paycheck to the address of residence, or court issued documentation) must also be submitted.

PROOF OF IMMUNIZATION

v Please bring original documents and one copy for school files

Ø 3 DPT; 3 OPV or 4 IPV, 3 Hepatitis B, 2 Measles, 1 Mumps, 1 Rubella, 1 Varicella, & Lead Screening

Ø Proof of immunizations and current physical must be signed and stamped by a physician.

Ø A physical examination report must be done within one year of registration.
