PROUDLY PRESENTS THE

Pre-Kindergarten Program  

A new world awaits the child who begins school.  To insure that this experience will be as rewarding as possible, we offer a voluntary program to all children who will be five years of age by December 1, 2011 and are eligible to be enrolled in our Kindergarten in September 2011.

 
This Pre-Kindergarten Program acquaints children and parents with their new school.  In addition, it begins to build the language and concept base necessary for success in school.  Games, stories, songs and a variety of readiness activities make this an exciting program for all.

All registrations will take place at the

Leon J. Campo Salisbury Center Registrar’s Office, 718 The Plain Rd., Westbury, NY

 on the following dates according to your child’s home school.

REGISTRATION TIME:     8:30 to 11:30 A.M. and 1:00 to 3:00 P.M.

Registration packets will be available at your school or on the district wegsite.

PROOF OF RESIDENCY AND ELIGIBILITY:

Please bring original documents and one copy for school files

An ORIGINAL BIRTH CERTIFICATE (translated if necessary), original baptismal certificate, if none available, valid passport with parental photo ID.

IF YOU OWN YOUR HOME, you must present your mortgage statement, property tax receipt and three current utilities bills. (e.g., phone bill, LIPA bill, water bill, cable bill, oil bill)

IF YOU ARE RENTING/LIVING WITH OTHERS, owner and non-owner affidavits are required (both can be downloaded from the district website). In addition, the owner of the property must provide you with the following: tax receipt and mortgage statement or home insurance and three current utility bills. You will also have to provide proof of address by presenting two bills received in your name at this address (e.g., phone, LIPA, bank statement, cancelled personal check with imprinted address or printed paycheck to the address of residence, or court issued documentation.

PROOF OF IMMUNIZATION:

3 DPT; 3 OPV or 4 IPV, 4 PVC, 3 Hepatitis B, HIB, 2 Measles, 1 Mumps, 1 Rubella, 1 Varicella, & Lead Screening

Proof of immunizations must be signed and stamped by a physician

A physical examination report must be done within one year of registration.

Health and Dental forms may be downloaded from the district website.      

PROGRAM DATES

February 7, 2011 – May 20, 2011

Twenty-four 1 ½ hour sessions to be held twice weekly. Days are determined by your child's home school. Pre-K will not be in session: 2/21-2/25 and 4/18-4/29.
